IBJI Terms and Conditions for IBJI Patient MyChart

Illinois Bone and Joint Institute, LLC offers MyChart as a method to provide patients a secure online environment to access their electronic medical record.  These Terms and Conditions outline our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

MyChart will provide you with access to confidential medical information, electronic communication with your provider, possibly website links to resources and proxy access.  An account will be established, and you will need to enroll using your MyChart ID.    This platform will request a valid email address and uses a password which you will need to manage and to keep secure.  You will be responsible for all activity in your Account.  If you need assistance with your account, contact the IBJI help desk at (847) 486-6900 or by email at support@ibji.com. 

 

Response to Electronic Communication.  We will make our best effort to provide a timely response to inquiries via MyChart, but you should allow at least two (2) business days for a response. If you request an immediate response or if you think you are experiencing a medical or psychiatric emergency, call 911 or your doctor immediately.

Physician offices and clinics are only able to respond to MyChart communications based on the information provided by you, the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services. Your communication via MyChart permits IBJI to respond to you via MyChart no additional communication is required.    

 

Web Site Links.   MyChart may offer links to related medical websites which are not managed by IBJI. These website link(s) are for informational purposes only. Your provider does not endorse and has not verified the accuracy of the information in/on these websites, and you should not rely on any of the information found on the web sites for purposes of treatment or diagnosis.

 

Minor Eligibility.  An individual must be 18 years or older to request a personal account. Minor children will not be eligible to participate in MyChart services. Parents or guardians of minor children may request access to the minor child's account by completing the MyChart Child Proxy form. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 12 years or older may not view without consent of the minor patient. Because of these requirements, we can only offer limited services to parents of minor patients in this age category.

 

Proxy Access. A parent or legal guardian of a minor child up to the child's 17th birthday may request proxy access to the minor child's MyChart account by completing and submitting the proxy access consent form. Such access will only be granted to parties with parental rights or legal guardianship over the MyChart account holder and only to the extent the party requesting proxy access can demonstrate the legal right to the account holder's medical information. If the proxy's legal relationship with the account holder changes, the proxy must inform the physician office or clinic immediately by calling the physician office or clinic. Individuals age 18 years or older may request proxy access to their MyChart account for another individual by completing the appropriate proxy access consent form and submitting it to the provider. A MyChart account will be activated for both the proxy and the account holder.  Your provider reserves the right to revoke proxy access at any time for any reason.  

 

Email Privacy.  As a user of MyChart, you will be notified via email when there is new medical information to be viewed in MyChart. This means that any person with access to your email will be able to see this notification. You should take this into account when providing an email address for MyChart.

If you send a message through your MyChart, it may be shared with your Provider's staff. 

Security and Confidentiality.  Providers afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by us in any other medium. We are committed to protecting the confidentiality of your medical information. 

MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser. You can determine if you are connected to MyChart by viewing the digital certificate. This certificate verifies the connection between your Providers server's public key and the server's identification.

As a MyChart user, your role in maintaining the security of your medical information is to: 1) Change your password on a regular basis and when you believe it may have been compromised in any way, and 2) Keep your login ID and password confidential. If you share your MyChart ID and password with another person, that person may be able to view health information in MyChart.

 

Access to information not included in MyChart.  MyChart is intended as a secure online source of confidential medical information for IBJI patients and their proxies. It contains selected, limited medical information from a patient's medical record and does not reflect the complete contents of the medical record. A complete paper copy of a patient's medical record may be requested from the patient's physician office or clinic.

Patients of providers that use Epic EMR software can connect third-party applications (apps) to retrieve portions of their health record for their own personal use. Examples of data that can be pulled into an app include lab results, allergies, medications, and immunization history.

To authenticate an app to retrieve your health data, follow these steps (note that images may not reflect your healthcare organization's exact layout or content for each step in the process):

  1. Make sure you have a MyChart (patient portal) account created for your healthcare provider organization. You will need your login credentials for the authentication process. Note: MyChart is an Epic branded name. Your organization may use a different name for their patient portal.
  2. Access the application on your personal device. Carefully review the app's terms and conditions.
  3. The app may ask you to select your healthcare provider from a list. Select your healthcare provider.

General Access Terms.  Access to MyChart is given by IBJI as a convenience to our patients and we have the right to deactivate access to MyChart at any time for any reason.

The use of MyChart is voluntary and you are not required to use MyChart or to authorize a MyChart proxy.

 

Legal Compliance.  IBJI users are prohibited from posting or transmitting any unlawful, threatening, libelous, defamatory, obscene, scandalous, inflammatory, pornographic, or profane material on the patient portal website. IBJI will fully cooperate with law enforcement authorities or court order requesting or directing IBJI to disclose the identity of anyone posting or transmitting content that encourages conduct that is considered a criminal offense, civil liability, or otherwise violates any law.

General Legal Terms

You agree to be bound by the following general terms:

  1. These Terms, along with any additional terms and conditions incorporated herein, constitute the entire understanding by and between IBJI and you with respect to the matters contained herein.
  2. Our failure to exercise or enforce any right or provision of these Terms shall not constitute a waiver of such right or provision. Waivers are effective only if in writing and signed by IBJI.
  3. IBJI may amend these terms at any time without notice to the patient.  
  4. If any provision of these Terms is or becomes unenforceable or invalid, the remaining provisions will continue with the same effect as if such unenforceable or invalid provision had not been used.
  5. These Terms ensure the benefit of, and will be binding upon, our and your permitted successors and assigns.
  6. You may not transfer any of your rights or obligations under these Terms to anyone else without our prior written consent. We may assign or delegate any of our rights and obligations under these Terms.
  7. A printed version of these Terms and of any related notice given in electronic form shall be admissible in arbitral, judicial, or administrative proceedings based upon or relating to these Terms to the same extent and subject to the same conditions as other business documents and records originally generated and maintained in printed form.
  8. You agree that we may send you notice via email to the email address you have provided, and we are not responsible for your failure to receive notice if email is quarantined by your email security system (e.g., "junk" or "spam" folder) or if you fail to update your email address. You also agree that we may send you notice through the Digital Services or your Account.
  9. Headings and captions are for convenience only.
  10. Use of the term "including" shall be construed without any limitation.

 

Agreement.  By accepting these Terms and Conditions, you acknowledge that you have read and understand this information and you agree to its terms.
